Heparin Sodium Flush

Heparin Sodium Lock Flush is a sterile, preservative-free or preserved solution of heparin sodium designed to maintain the patency of indwelling intravenous catheters. It is a low-dose anticoagulant solution intended only for local catheter instillation and is not to be used for systemic anticoagulation therapy.

Dosage by Body Weight

The recommended dose is 10 to 100 USP units (typically 1 to 3 mL), instilled into the catheter lumen after each use or every 8 to 24 hours. The volume of the flush should equal or slightly exceed the internal priming volume of the catheter.

  1. Body Weight
    Below 5 kg
    Dose
    10 units (1 mL of 10 units/mL)
    Frequency
    After each catheter use
    Tracking Notes
Use extreme caution in neonates and low birth-weight infants. Use only preservative-free 10 units/mL solution to avoid benzyl alcohol toxicity and systemic anticoagulation.
  2. Body Weight
    5-7 kg
    Dose
    10 to 100 units
    Frequency
    After each catheter use
    Tracking Notes
Volume instilled should match the catheter's priming volume to avoid infusing excess heparin systemically.
  3. Body Weight
    8-10 kg
    Dose
    10 to 100 units
    Frequency
    After each catheter use
    Tracking Notes
Volume instilled should match the catheter's priming volume to avoid infusing excess heparin systemically.
  4. Body Weight
    11-15 kg
    Dose
    10 to 100 units
    Frequency
    After each catheter use
    Tracking Notes
Adjust volume to equal the specific indwelling device volume (typically 1 to 3 mL).
  5. Body Weight
    16-20 kg
    Dose
    10 to 100 units
    Frequency
    After each catheter use
    Tracking Notes
Adjust volume to equal the specific indwelling device volume (typically 1 to 3 mL).
